Wellness Program

Valir is at the forefront of the nationwide campaign to increase geriatric physical activity and it is our belief that a wellness program must be an integral part of any comprehensive rehabilitation program. We believe that physical activity is the best defense, and offense, in preventing illness, disease and aging symptoms, and healthcare workers are often the primary motivators in encouraging older adults to remain physically active.

The goal of our CORE wellness program is to promote functional independence and delay the effects of aging by improving overall health and well-being. We reach this goal by providing facilities and their residents with evidence-based, individualized wellness regimens.

The CORE program combines the four-tiered model in the American Physical Therapy Association's "Active Aging Toolkit" (published in the Journal of Geriatric Physical Therapy) with three phases of wellness. Our experienced therapists begin by assessing the wellness of the residents indentified by your team. We then develop wellness strategies based on these assessments, educate the residents, and monitor and assist their progress.

"CORE" is an acronym containing the following four components:

These core elements are then integrated with the four tiers of the Active Aging Toolkit:

  • Endurance
  • Flexibility
  • Strength
  • Balance

Our CORE program also incorporates the following:

  • Medically necessary skilled rehab needs
  • Evidence-based prevention and functional maintenance
  • Education

Often, Valir will be able to submit resident assessments to insurance providers for payment. If they qualify for medically necessary skilled therapy, a claim will also be submitted to their insurance carrier. Development and ongoing assessment of the individual's program usually will not be billed to the resident or facility. However, in those cases in which extensive assistance is routinely needed, payment may become necessary.

Home Health

Valir Health offers home health services to residents once they return to their home from a facility.  We understand the challenges in remaining independent at home, and we strive to make a patient's independence feasible and safe.

Community Education
Valir believes that education and empowerment, especially in the aging community, are vital to extending functional independence and promoting improved health and wellness. We are involved in many different community groups and offer educational programs to:
  • Support groups
  • Churches
  • Retirement communities
  • Schools
  • Health fairs
